This release form covers all the bases and can be downloaded or printed from right HERE!
For all events we engage in that happen off-site (not @SLW), this form is essential.
It covers liability and safety, and is designed to protect all parties involved.
Keep in mind, if you have filled this form out for another activity, it is not necessary to fill out the contact or emergency information (unless it needs updating).
However, parents/guardians (or 18+ students) will need to sign the lower portion of the form for each event.
